WHAT IS MEDICATION RECONCILIATION?

This is the process by which you find out a patient's current medication list,  ensure that a patient gets the medications that he was taking at home, determine if they should be continued in the hospital and help decide what to send a patient home with at discharge. 

You will obtain a medication history by calling the pharmacy, speaking to the patient or family member.

After you do this you need to write a note in the electronic medical record (PRISM) in Documenter. This is what a sample note should look like:
